Your Medical Records

You can access your medical records online any time through our patient portal. Your patient portal account includes test results and other details about your medical care. 

At Winchester Hospital, the office of Health Information Management maintains patient medical records. The office is not open to the public for in-person visits.

How to Request Copies of Medical Records

You can call the Health Information Management at 781-756-2170 to request copies of your patient medical records. The office is open Monday through Friday from 8:30 am to 4:00 pm. (except holidays).

If you need a copy of your medical records sent to another medical facility or for personal reasons, you must make a written request. This allows you to authorize us to release your records.

Charges may apply for copies of medical records. If there is a fee, we will bill you or contact you in advance.

Write us a letter or download and complete this medical release form. If you write to us, please include this information:

• Your name and address
• Your date of birth
• The time (dates of service) for which you need your records
• The name of the person to receive the medical records
• The reason you need the medical records

Requesting Bills, Imaging or Records in an Emergency

If your records are needed in an emergency, your doctor can call Health Information Management at 781-756-2170.

• To request imaging films, call 781-756-2342.
• To request copies of your hospital bills, call 781-744-8772.

Birth Certificates

If you or your child was born at Winchester Hospital, you can request a copy of the birth certificate. To request a copy of a birth certificate, call or visit the Winchester Town Clerk's office:

While Winchester Hospital collects information included on birth certificates, the certificates are town records and are not available through the hospital.
